Appointments
We try to see our patients on an appointment basis, and we request that you call in advance so that we can reserve time for you. We make every effort to honor all time commitments, but on occasion, if there will be a delay or an emergency, you will be informed, so we can reschedule at a time convenient for you.
If you need to cancel, please do so at least one day prior to your appointment.
What to Bring
- Referral letter from your general practice physician, family physician or other doctor.
- Completed Patient Information form.
- Your insurance information.
- Copies of operative reports, medical records, X-rays, MRIs, CT scans, etc, from prior doctor visits.
- If you have had surgery elsewhere, please bring a copy of your operative report.
